Health Promotion & Disease Prevention
We want to provide healthy living opportunities to our Veterans, and this is one of the ways we can help serve those who have already served for us!
The Preventive Care Program is designed to provide Veterans with comprehensive health education, clinical services, health behavior change coaching and support to help them to enhance self-management of their health conditions. Improving health behaviors is often not easily done. The Program is closely aligned with the Patient Aligned Care Team (PACT) initiative and provides resources to support facility HPDP (Health Promotion & Disease Prevention) infrastructure. These include staff training and coaching as well as tools to assist PACT and other clinical staff in assessing and addressing Veterans’ health promotion and disease prevention needs and interests.
To encourage Veterans to choose healthy behaviors, nine key evidence-based healthy living messages have been developed. Each message includes a brief overall message with more detailed information for those Veterans who are interested. These messages are posted on the NCP (National Center on Health Promotion & Disease Prevention) website: www.prevention.va.gov.

Life Goal Initiative
The Walla Walla VA wanted to enhance Personalized, Proactive, and Patient Driven Care by piloting the Life Goal Initiative, which was implemented at Walla Walla VA's Richland CBOC. Walla Walla VA was subsequently recognized by National Primary Care as the winner of the PACT Best Practice in the nation for this innovative practice. The Life Goal is a process that takes the principles of TEACH for Success and Motivational Interviewing and more fully utilizes these principles with our Veterans to assist them to become more involved in their health care, among other things. The primary purpose of the Life Goal is to find out what matters to the Veteran, establish a relationship with said Veteran and/or the family, and build care around these desires.
